  • Care Coordination Add-On: +$10/hour when bundled with an active care plan

  • Advocacy Consultation: $75 for a standalone 60-minute session

  • Monthly Coordination Package: $199/month (up to 4 hours of dedicated coordination support)

  • Discharge Planning Support: $199 flat fee (up to 3 sessions for post-hospital or post-surgery transitions)
